#5524 THE ROLE OF C5AR VERSUS C3 IN THROMBOTIC MICROANGIOPATHY, ISCHEMIC KIDNEY NECROSIS, AND ACUTE KIDNEY INJURY INDUCED BY CHOLESTEROL CRYSTAL EMBOLISM

چکیده
Abstract Background and Aims Certain thrombotic microangiopathies (TMA) respond to inhibition of complement factor C5, which abrogates formation the membrane attack complex hence impairs host defense. Inhibition C5a receptor (C5aR) avoids this safety problem still controls autoimmune systemic vasculitis. We speculated that targeting C5aR could be sufficient also in TMA, e.g., related cholesterol crystal embolism (CCE). Method induced experimental TMA C3-/-, C5aR-/- or wildtype mice by injecting CC into left kidney artery analyzed angiopathy, drop measured glomerular filtration rate (GFR), ischemic necrosis 24 hours after CCE. Results In mice, injection caused diffuse followed a consistent GFR compared baseline necrosis. Genetic deficiency C3 convertase profoundly attenuated indicating key role system CCE-related TMA. same extent provides main contribution pathogenesis Conclusion conclude C5a/C5aR axis attenuate for better profile would expected as C5 inhibition.

Reduction of hemorrhagic shock–induced acute kidney injury by lower limb ischemic preconditioning in rats
Introduction: During hemorrhagic shock (HS), the kidneys are one of the primary target organs involved. Oxidative stress is shown to be enhanced in different models of acute kidney injury (AKI). Remote ischemic preconditioning (RPC) by brief limb ischemia is considered to be a safe method to protect different organs from further damage. In this study, we investigated the effects of brief hind l...
متن کاملAcute kidney injury induced by thrombotic microangiopathy in a patient with hemophagocytic lymphohistiocytosis
BACKGROUND Hemophagocytic lymphohistiocytosis (HLH) is a fatal clinical syndrome characterized by excessive immune activation and inflammation. It is frequently complicated by acute kidney injury (AKI) that often develops as acute tubular necrosis (ATN). Meanwhile, renal thrombotic microangiopathy (TMA) is a rare pathologic finding that mostly occurs in hemolytic uremic syndrome or thrombotic t...
متن کاملCholesterol Crystal Embolism and Chronic Kidney Disease
Renal disease caused by cholesterol crystal embolism (CCE) occurs when cholesterol crystals become lodged in small renal arteries after small pieces of atheromatous plaques break off from the aorta or renal arteries and shower the downstream vascular bed. CCE is a multisystemic disease but kidneys are particularly vulnerable to atheroembolic disease, which can cause an acute, subacute, or chron...
متن کاملThrombotic microangiopathy after kidney transplantation.
Thrombotic microangiopathy (TMA) is a severe complication of kidney transplantation that often causes graft failure. TMA may occur de novo, often triggered by immunosuppressive drugs and acute antibody-mediated rejection, or recur in patients with previous history of hemolytic uremic syndrome (HUS). Recurrent TMA is very rare in patients who had developed end-stage renal failure following HUS c...
